List managers for a picker

GET /api/v1/managers/all prm_managers_index

Id and name only, un-paginated, for populating a select control. Same subtree scope as the full list. The workspace owner is left out, as in the full list; with_owner=1 adds them for a picker that attaches something to a manager, with their email blank.
